If you can lose the weight, but can’t keep it off…
That pattern is extremely common in midlife.
Weight changes in perimenopause and menopause aren’t just about willpower. Hormonal shifts can affect appetite regulation, insulin sensitivity, sleep, stress levels and body composition — even when you’re eating well and staying active.
Our care focuses on long-term stability, not short-term wins. We help you understand what may be driving your weight changes, and support you with safe, clinically guided treatment where appropriate.
Many women notice shifts in weight, appetite, energy levels or body composition during perimenopause and menopause. These changes are influenced by a combination of:
- Hormonal fluctuations
- Changes in metabolism
- Altered appetite regulation (“food noise”)
- Sleep disruption
- Mood and stress levels
- Loss of muscle mass
- Lifestyle patterns that may no longer feel effective
For many women, these changes happen despite healthy eating, regular activity and good intentions. Our doctors help you understand what may be driving your individual changes — and how to support your health safely and effectively.
Weight management injections (often called GLP-1 medications) are prescription treatments that support appetite regulation and help reduce persistent hunger and cravings.
For many women, they can help quiet the “food noise” — making it easier to eat in a way that feels steady, sustainable, and less exhausting.
These medications aren’t right for everyone, and they’re not a quick fix. That’s why our doctors assess your health carefully, prescribe only when appropriate, and monitor your progress over time to ensure treatment remains safe and effective.
These treatments are licensed for:
- Individuals with a BMI of 30 or above, or
- Individuals with a BMI of 27 or above with at least one weight-related health condition
Weight-related conditions include (but are not limited to):
- Pre-diabetes
- High blood pressure
- High cholesterol
- Cardiovascular disease
- Sleep apnoea
- Osteoarthritis
- PCOS
- Fatty liver disease
People of South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds may be eligible at lower BMI thresholds due to higher health risks:
- BMI of 27.5 or above, or
- BMI of 24.5 or above with a weight-related health condition
We cannot prescribe if you have:
- A known allergy or previous reaction to GLP-1 medications
- Heart failure, severe liver or kidney disease
- A history of pancreatitis
- Multiple endocrine neoplasia type 2 (MEN2)
- A personal or family history of medullary thyroid cancer
- Type 1 diabetes
- Type 2 diabetes where you are currently on diabetic medication
Other conditions may require additional discussion. Your doctor will assess your circumstances carefully to ensure treatment is safe.

Everything you want to know
Weight changes in midlife are common and not simply about willpower. Hormonal fluctuations — particularly changes in oestrogen — can affect appetite regulation, insulin sensitivity, sleep, muscle mass and fat distribution. Many women notice that strategies that worked in their 20s or 30s feel less effective. Our role is to help you understand what may be influencing your individual changes and explore safe, appropriate treatment options.
There is no strong evidence that HRT directly causes weight gain. In fact, stabilising hormones can sometimes support sleep, energy and muscle maintenance, which may indirectly help with weight stability. Weight changes during midlife are often due to hormonal shifts and metabolic changes rather than HRT itself. Your doctor can help assess what may be contributing in your specific case.
HRT is not a weight loss treatment. However, by improving sleep, mood and energy, some women find it easier to maintain healthy routines. If weight is a significant concern, it’s important to assess metabolic health separately and consider whether additional support — including weight management medication — may be appropriate.
GLP-1 medications are prescription treatments that help regulate appetite and improve feelings of fullness. They work on pathways involved in hunger and blood sugar regulation. Many women describe the biggest change as a reduction in persistent cravings or “food noise”, making eating feel calmer and more manageable. These medications are not suitable for everyone and require medical assessment and ongoing monitoring.
When prescribed appropriately and monitored by an experienced clinician, GLP-1 medications can be safe and effective for eligible individuals. We assess your full medical history, current medications and risk factors before prescribing. Ongoing review ensures that treatment remains safe and appropriate for you.
Weight regain can occur after stopping any weight management medication if no longer supported by appetite regulation and lifestyle adjustments. That’s why we focus on long-term metabolic health, sustainable habits, and careful review rather than short-term results. Your doctor will discuss realistic expectations and long-term planning with you.
In many cases, yes, but it depends on your individual health profile and medications. Our doctors assess safety carefully and ensure that any prescribing fits safely alongside HRT, contraception or other treatment.
No. Our approach is focused on long-term stability and medical safety. Medication is one tool that may support appetite regulation, but it works best alongside structured medical review, lifestyle support and ongoing monitoring.
Preserving muscle mass is important, particularly in midlife. We encourage strength-based activity and adequate protein intake to support muscle health while managing weight. Your clinician can guide you based on your individual needs.
Weight management injections are licensed for specific BMI thresholds. If you do not meet these criteria, your doctor can still assess metabolic health, hormone balance and other factors contributing to weight changes, and discuss alternative strategies where appropriate.
Very normal. Weight is deeply personal, and many women feel they should be able to manage it alone. Midlife hormonal changes can make weight regulation more complex than it has been before. Seeking medically guided support is not a failure, it’s a decision to prioritise your health.
